The Phone Call That Should Not Exist

It is 2026, and the way most dental practices interact with their IT support still follows a pattern that was established in 1998.

Something breaks. Someone at the front desk picks up the phone. They describe the problem as best they can. A ticket is created. A technician eventually connects remotely — or, in many cases, drives to the practice. They diagnose the issue, apply a fix, and close the ticket. The practice receives a bill.

This model has been refined over the decades. Managed Service Providers replaced break/fix billing with monthly contracts. Remote monitoring tools replaced some site visits with remote sessions. But the fundamental architecture has not changed: your practice experiences a problem, and then a human responds to it.

In virtually every other technology-dependent industry — manufacturing, logistics, financial services, telecommunications — this model was abandoned years ago in favor of systems that prevent problems from occurring in the first place. The dental industry is the last major sector still running on reactive IT.

That is changing.

The Four Stages of Dental IT Maturity

To understand where the industry is headed, it helps to understand where it has been. Dental IT has progressed through four distinct stages — and most practices are still stuck at stage two.

Stage 1: Break/Fix (1990s-2000s)

The original dental IT model. No monitoring. No maintenance contract. When a computer breaks, you call a technician who charges by the hour. Every outage is a surprise. Response times are measured in hours or days. Your IT relationship is entirely reactive — you only interact with your technology support when something has already gone wrong.

Stage 2: Managed Services (2005-2015)

The managed service model introduced monthly contracts, remote monitoring tools, and scheduled maintenance visits. Your MSP installs an RMM (Remote Monitoring and Management) agent on your machines and monitors basic health metrics: CPU usage, disk space, Windows update status. When a threshold is exceeded, a ticket is created and a technician investigates.

This was a genuine improvement over break/fix. Practices gained the benefit of proactive maintenance — patch management, backup monitoring, and periodic security reviews — without needing in-house IT staff.

The limitation: Managed services shifted the billing model from reactive to predictable, but the response model remained fundamentally reactive. When your PMS crashes, the MSP's monitoring tool detects the crash and creates a ticket. A human technician must then review the ticket, remotely connect, diagnose the issue, and apply a fix. The monitoring is automated; the response is not.

Where this stage lives today: The vast majority of dental practices that contract with an MSP — including dental-specialized MSPs — operate at this stage. It is the current industry standard.

Stage 3: Proactive Monitoring (2015-2023)

The proactive stage adds predictive capabilities to the managed service model. Instead of just reacting when a threshold is exceeded, the monitoring system identifies trends: a disk that is filling at a rate that will cause problems in two weeks, a database that is growing beyond its storage allocation, a backup that has been incrementally failing for the past three nights.

Proactive monitoring reduces the severity and frequency of disruptions by catching issues in their early stages. But it still relies on a human technician to interpret the alerts and take action.

The limitation: "Proactive" is one of the most overused words in dental IT marketing. Nearly every MSP claims to offer proactive monitoring. In practice, this typically means the technician reviews a dashboard of alerts each morning and prioritizes the most urgent ones. Low-priority alerts — the kind that predict problems days or weeks out — are frequently deprioritized in favor of the day's active fires.

Where this stage lives today: The best dental MSPs and a handful of specialized dental IT companies genuinely operate at this stage, though the depth and consistency of their proactive monitoring varies significantly.

Stage 4: Autonomous Remediation (2024-Present)

This is the stage that changes everything.

Autonomous remediation eliminates the human delay between detection and response entirely. When the monitoring system identifies a problem — a crashed service, a disconnected network drive, a corrupted printer spooler, a failing database connection — it does not create a ticket and wait for a technician. It classifies the issue, selects the appropriate remediation procedure, executes it, verifies the resolution, and logs the entire event.

No phone call. No ticket. No wait. No human in the loop for the categories of problems that have well-known, well-documented solutions.

What this means in practice: Your Open Dental eConnector fails at 6:43 AM. By 6:44 AM, it has been restarted and verified operational. Your morning schedule loads normally. Nobody in the practice ever knows there was an issue — unless they check the automated incident log.

They understand imaging systems, dental databases, and HIPAA compliance requirements. They represent the current gold standard in dental IT support.

The limitation is not expertise — it is architecture. Even the best dental MSPs are constrained by the managed service model: detect → ticket → diagnose → respond. The expertise is there. The speed is bounded by the human response time.
