The independent practitioner's response to DSO consolidation falls into one of two categories: compete effectively, or exit strategically. Both are viable. But doing neither — continuing to operate as if the competitive landscape hasn't fundamentally changed — is a path to declining revenue, shrinking patient base, and an eventual fire-sale exit at a fraction of what the practice could have been worth.

Understanding What DSOs Actually Buy

DSOs don't buy dental practices for the dentistry. They buy cash flow, patient volume, and operational leverage.

The spread between those multiples — nearly $800,000 — is determined entirely by the practice's operational quality and strategic positioning in the two to three years before the sale.

The Exit Strategy: Maximizing Valuation

Practice owners considering a DSO exit within the next three to five years should begin optimization now. The changes that increase valuation take time to mature, and DSO buyers scrutinize trends, not snapshots.

Diversify production across providers. A practice where one dentist generates 90% of production is a key-person risk. Adding an associate, expanding hygiene hours, or bringing in a specialist one day per week distributes revenue across multiple providers — making the practice more attractive and the transition less disruptive.

Expand the service mix. Practices offering implants, clear aligners, pediatric services, or oral surgery command higher multiples because they capture more revenue per patient and reduce referral leakage. A general practice that adds implant placement and guided surgery can increase case values by 30% or more while demonstrating growth trajectory to buyers.

Clean up the books. DSO buyers conduct detailed financial due diligence. The Competition Strategy: Winning as an Independent

For practice owners who have no interest in selling — and there are many — the response to DSO competition requires a clear-eyed assessment of where independent practices hold structural advantages.

Continuity of care. DSOs experience high provider turnover. Patients who value a long-term relationship with their dentist — particularly older patients, complex treatment cases, and families — will choose continuity over convenience. Marketing that emphasizes "same dentist, every visit" directly targets the DSO's most significant weakness.

Clinical autonomy. Independent practices can offer treatment plans based purely on clinical judgment, without corporate production targets, approved materials lists, or preferred laboratory mandates. For treatment-savvy patients (and they're increasingly savvy, thanks to the internet), the difference between "your dentist recommends this treatment" and "corporate headquarters approved this treatment plan" is meaningful.

Community integration. Independent practices can build local brand equity that DSOs structurally cannot replicate. Sponsoring youth sports teams, participating in community health events, maintaining relationships with local physicians and specialists — these activities create referral networks and patient loyalty that survive competitive pressure.

Operational efficiency. Independent practices that invest in modern technology (cloud-based PMS, AI diagnostic tools, same-day restorations, automated patient communication) can match or exceed DSO operational efficiency without the corporate overhead. The technology that DSOs deploy at scale is available to independent practices at price points that have dropped dramatically over the past three years.

Membership plans. As covered elsewhere in this issue, membership plans allow independent practices to build recurring revenue streams that reduce insurance dependency and increase per-patient profitability — advantages that DSOs, which typically accept every insurance plan to maximize patient volume, are structurally unable to replicate.

The Hybrid Path

A third option is gaining traction: the invisible DSO partnership. Some DSO models offer management services agreements (MSAs) where the practice retains clinical independence and branding while the DSO handles billing, marketing, HR, purchasing, and compliance.
